How to join our Payment Plan
Simply call in at the practice or complete the enquiry request form below and we will contact you.
We will provide you with an agreement form which will require your signature and bank account details for the Direct Debit Mandate.
Direct debits are collected from your account on the 8th of every month. Any patient enrolling before the 27th day of each month will benefit from cover from the 8th of the following month.
